A three-year-old male patient presented already at his birth a disproportion macrosomia of the left foot and a large, nodular nevus flammeus in the left hip region, which led to the tentative diagnosis of a Klippel-Trenaunay syndrome. In the following years, both changes showed a continuous progression, with distinct soft-tissue swelling as well as papillomatous and verruciform vegetations of the nevus. Additionally, large, plain subcutaneous masses developed under the right shoulder, and a macrodactyly of the first and second left toe could be observed. Although several examinations had been performed in the meantime, the tentative diagnosis could not be confirmed up to that time. On the occasion of a severe local infection in the hip region, which led to the consideration of a surgical therapy, a radionuclide lymphography, a blood pool scintigraphy including dynamic phlebography and ventriculography as well as a bone scintigraphy were performed. These examinations were done simultaneously at one day in order to avoid a longer period of immobilization. The findings led to the diagnosis of a large lymphangioma, which could be confirmed histologically after surgery. In consideration of all results, the basic disorder seems to be the rare proteus syndrome rather than a Klippel-Trenaunay syndrome.

Human skin can be colonized by different yeasts that may have an impact on skin pigmentation. In order to study this effect normal human melanocytes were cultured with different yeasts. Reverse transcription polymerase chain reaction (RT-PCR) analysis gives evidence that Candida albicans suppresses the transcription of melanogenesis enzymes.

Faeces samples taken from 343 patients with psoriasis and 581 patients with atopic dermatitis were subjected to mycological examination. Yeasts were detected in 68% of the psoriatics and in 70% of the patients with atopic dermatitis but in only 54% of the controls (n = 50). Qualitative analysis revealed a predominance of Candida albicans. Non-pathogenic yeasts constituted only 1% in each of these groups. Of the hyphomycetes, Geotrichum candidum occurred in 22% of the psoriatics, in 10% of the atopic dermatitis patients and in 3% of the controls. Aspergillus species were found in 1% of the patients but not in the controls. Stool samples collected on three consecutive days from 141 patients were examined for yeasts. Qualitative correlation between all three samples was shown in 95% of the patients and quantitative correlation in 38%. Deviations were mainly of exponential magnitude. Germ cell concentration of 10(4) cells per ml and above were measured in 38% of the psoriatics and in 28% of the atopic dermatitis patients but in only 22% of the test subjects with healthy skin. There was no correlation between the concentration levels of yeasts in the faeces and the extent of psoriasis or atopic dermatitis.

Reactions induced by neurotoxic effects with lindane (gamma-hexachlorcyclohexane) after treatment of infants have been reported by different authors. This has led to controversial discussions as to the indication for therapy with lindane in early infancy. To our knowledge there are to date no published reports on the concentration of lindane in mother's milk after local scabies therapy. In a mother treated with lindane for scabies we found elevated levels of lindane in the mother's milk. Compared to average levels of lindane in mother's milk (0.03 ppm, milk-fat) there was a 30-fold increase after three-day treatment and a 66-fold elevation after an additional one-day treatment with lindane. The problems arising and alternative therapy are discussed.

Stool samples of patients suffering from seborrheic eczema of the scalp, psoriasis capitis, or seborrhiasis were investigated. All patients revealed pathological flora of the bowel to a high degree, predominantly pathogenic yeasts. Therapy of the intestine seems to be of good influence to the skin disease as well.

We discuss atopic dermatitis in relation to candidamycosis of the intestine. In 11 out of 36 patients with atopic dermatitis, we found specific Candidin-IgE-antibodies. With all patients, candidamycosis of the stool samples was positive in culture. Increasing incidences of both diseases support the clinical relevance of these observations.

Some aspects in the development of severe tissue damage after an adriamycin extravasation are considered as a sympathetic reflex dystrophy syndrome. In this, a peripheral stimulus (e.g. pain) activates efferent autonomic and motor nerves resulting in vasomotor changes and other "dystrophic" changes in soft tissues. A typical state is the Sudeck's dystrophy syndrome. With a transcutaneous electrical nerve stimulation it should be possible to interrupt this vicious circle. A case with a good clinical result is reported.
